Shipping by Distance Considering Weight

HI,
I want help regarding the following issue.

I want to implement shipping by distance but consider the weight as well. for example, $5.00 for first 100 KM. And $7.00 for more than 100 KM. Besides this, I want to add another cost for weight. Let say $0.5/KG . no matter what distance.

in short; if some one from 100 km distance order a product of 2 kg, shipping charge will be : $5.00 + $ 1.00
