Setup with MailChimp delivers 404 page

Hello

I’m using this plugin with MailChimp and it isn’t working. When an email address is entered a 404 page is displayed and no emails go into my mailchimp list. I went through your tutorial.

I copied the code for the first part and inputted into the “form action” field in the same format as below:

<form action=”https://brandonkraft.us2.list-manage.com/subscribe/post?u=8bf735574fe7445970000d814&id=f942c3ccf5″&gt;

I inputted this code into the “E-Mail Field” field:

<input id=”Email” type=”text” />

Is any of the code above is incorrect?
