Sermon Notes Upload stopped working

When uploading a pdf as notes file for a sermon.

File uploads and appears on single sermon page but not on archive page. And the sermon notes field is empty.

The Media Upload dialogue is blank after hitting the “sermon notes” button.

There is the following js error:

Cannot call method ‘replace’ of undefined
