SENT notification: Java Script Function not firing

The “Sent” notification (via Java Script) isn’t firing when the form it sent. I’ll get multiple emails from people hitting the “send” button multiple time. What should I be looking for to fix this?
Also.. an idea to add… JS for Credit Card validation?
