When creating your own short code using Short Code Exec PHP, place the code for the short code in the administration page under Tools -> Shortcode Exec PHP. You give the short code a name there. Place your new short code on any WordPress page or post.

Short codes cannot verify login and password. But you might find Security Settings useful as well as the role attribute on short codes.
