Senders FROM address is not being captured

My form has 4 fields that the sender is required to complete: name, email, phone and message.
Once the email is received, the FROM address is defaulting to the website mailing address, and the sender’s email and phone do not appear at all. So we are not capturing the any contact info that the sender is providing.
