Send File to Custom Field Email

  • Hello, sorry for my english.
    I have a website in wordpress make available where some audio files.
    I would like to create a function where people could send a specific audio file, chosen for her, for your email.
    I need to create a form with an inbox and a “submit” button. This form also needs to “capture” the information within a determinadoo audio file located inside a custom field in the post since it is here that contains the audio file that will be attached to the email.

    Anyone know how do I create a form with these functions?
