    I just installed and activated this. To begin, I created a simple form with only a Select field. I configured the Select field to be a column from a table in a MySQL database. During the configuration, it accepted my hostname, username, password, and database; found the table; and allowed me select the column. After saving, the form appears under Form Maker | Forms. When I view it, it shows label “State” and a select box with “[States:Name]” which is the table:column from my database. But when I preview or load from a live page, all I get is the label and an empty select box. What am I doing wrong? Thanks.

    If we understand you correctly, you are connecting to Remote database. Please make sure that the database is not private or it doesn’t have any limit for permissions.
