Seeking password reminder plugin with a difference

Hi everyone,

I am seeking a password reminder plugin with a difference. I run a football referees website and everyone who referees has a PIN that they need to put on match cards at the end of games. I have a record of everyone’s PIN in a spreadsheet/database and every season I get inundated with email requests from referees who have forgotten their PIN during the off season. Does anyone know of a plugin that I could use that would enable me to build a simple online reminder form? The idea would be that I would upload a spreadsheet/database with various fields – name, date of birth, registered email address, suburb and so on – and that the plugin would email the PIN to the forgetful referee if he or she inputted matching information.

I found dozens of WordPress user password reminder plugins but I could not find anything that would service my special needs.
