Much appreciated, do you know of one that each secret shopper is able to have an account, and it keeps the stats from each time they go shopping? I browsed through them, but was unable to find one that would do what I’m looking for, thanks!

This is easily done with Formidable Pro (the non-free version). It maybe doable with other form plugins, too.
