• When I search using the box at the top right of the media library, it appears to look in caption, description, title and filename. However, it doesn’t seem to look at alt-text. Is there a reason for that, or is it an oversight?

That is because the alt text is a post meta, while the caption, title, and description are post data.

In other words, in the database those details are stored in different tables (postmeta and posts respectively)

The WordPress query that runs the Media search page only queries the posts table, and thus, postmeta will not be searched.
