Very dynamic form

  • Hello everyone. I’m looking for plugin/plugins of form that I could configure the following things:

    • Form takes default values from URL (parameters)
    • Some inputs/dropdowns (elements in general) are displayed conditionally (e.g. if something is selected display only these 3 specific inputs

    Nice to have:

    • Handling pages/steps in the form

    Any ideas?
