    I can see you are using the cache on your site https://www.godental365.com so could you please temporary disable it so that I can troubleshoot the issue?

    Have you custom developed the Go Dental 365 WordPress Theme that you are using on your site?

    This can be due to plugin conflict on your site so please try temporary deactivating all plugins except Add Search To Menu plugin and see whether everything works fine and then enable the plugins one by one to see which plugin is conflicting if any.

    Also this can be Go Dental 365 WordPress Theme issue on your site so to confirm it just temporary use the Twenty Seventeen theme instead of Go Dental 365 WordPress Theme on your site. If everything works fine then it’s your Go Dental 365 WordPress Theme issue.

    Would you know of anyhthing that could conflict with the plugin? I’ve tried deactivating all of the plugins but that didnt seem to help. Ill have to do some digging in my theme to see what might conflict.

    Sorry for the late reply. I missed this topic notification.

    I don’t know any specific conflicting plugin but as you have already tried deactivating all of the plugins then now you can try using default WordPress theme or setting plugin Search Form Style option to Default in the theme settings.
