Search for plugin that can manage advanced additional fields for members

Hi everyone,

Does anyone knows a plugin that can do this:

I’m want to create a wordpress website that dedicated for teachers to reveal information about students-

Teacher have to register the website, than when teacher user has an account they’ll have an option to fill some additional fields on their students like :
Student ID number, Student name, Student behavior rating.

The Teacher would be able to fill unlimited groups of admin made additional fields because probably the teacher will have more than one student.

Also student ID number should be unique so if another teacher will fill information on existing student, his rating will be update but other additional fields will keep same.

one more thing, Admin should approve the Teachers (members) for updating their information in the website.
