Search Field Directory

Please help! I have set up forms and tested them and when I test them the Search field works fine, but later after some entries have been made, they will only search one field. I have no clue what is causing this. I have trashed my forms and recreated them from scratch and they were working just fine and it does not make sense that the Search field will eventually stop working. Please advise. Thank you.
