Same thumbnail on all videos placeholders

Hi

I seem to be experiencing an issue where the same thumbnail is being shown on all video placeholders

I have cleared every cache known to man, woman or beast and its hasn’t made a difference

Any idea why this is happened? And more importantly, how to resolve?
