Same form on different pages

Hi

I have a form that took a while to get working but it was working well. I have subsequently embedded this single form on multiple pages within my website and now it seems the form is no longer hiding conditional fields when needed.

I have tried duplicating a version of the form but it also does not work.

How do I fix this issue? It doesnt seem to fall under wp_footer issue, nor javascript issues.

Do I need to perhaps recreate the form for each page that it is required as an embed and rename all the fields in each form to be 100% unique from any other version of the form throughout the website? This would be quite difficult and time consuming so I just wanted to check if there is another solution?

Ideally, i need to be able to embed the same form on multiple pages.
