Rewrite Only HALF Working

It seems that even after checking “force rewrite titles” in the Yoast admin area WordPress still inserts the site name and description into the title tag.
I say it’s only “half working” because the Yoast plugin is writing it’s part but WordPress is still writing the titles to the right of it.
Is this a plugin issue? Also, is there somewhere in the header where I can delete/modify some code to stop WordPress from doing this?
