Review order page – wp gift cert plugin

Hi I have a website using the elegance theme.

I have installed a plugin wp gift cert under the heading gift certificates. When I click on purchase cert it shows this review order page https://www.vitalitycolonics.co.uk/gift-vouchers/# which is completely screwy. How do I edit this review order page?
