Results to be viewed publically

Hello
I have created this form and I need this form once filled/submitted to be printed onto a page which is linked to that members profile or to a button which opens once clicked on.

At the current moment when I submit I have no idea where the details are sent because I can’t find where it’s going.

Also I would like that displayed page to change the input as the member updates it. Is that possible?

Help would be appreciated.
