Restoring my plugin

Hello,
I’m not sure what happened but my Sign up plugin on my site https://www.leahmarieblog.com looked like this (see screenshot) then suddenly the boxes moved and since I don’t know code and someone else set that up, I’m stuck. Can someone help me?
