Required Fields Missing

I’ve made this work with Mailchimp in the past, but now am having trouble with Aweber.
If I fill out the form on my site, I get taken to a pop-up:
“Required Fields are Missing”
I’ve double checked by watching two tutorials on filling out the widget, but I always get this message. I would also like to have it nicely display horizontally across the page. But, that may be a separate ticket. It’s for my client site, https://thesecondhalfclub.com.
