Request Warranty Function Not Working

Everything about this plugin works the way I need it to, except for the Request Warranty function. When clicking on the Request Warranty button on the My Orders screen, it tries to redirect to the warranty request form, however it leaves out a slash between “new-warranty-request” and the order number:

/my-account/new-warranty-request72196

So it pulls in a 404. When I manually add the slash to the url it works:

/my-account/new-warranty-request/72196

How do I fix this problem?
