Request a Quote pg html code showing

Runing WordPress 4.4.2, WooCommerce 2.5.5
Using theme Gather.

Hello,
Installed the plugin without issues. After clicking the request button and then viewing the Reuest for Quote page there was odd html/javascript showing in and below the email field.

Has anyone had this issue or know of a fix?

In the email text field it said:
<span id= wp-usename
and then below the field it said:
email hidden; Javascript is required wp-user@emailaddress" required>

note: wp-username and wp-user@emailaddress in the above are just placeholder.
