"report bug" pop-up is blank

I’m using WordPress 3.6 and the latest version of Bug Library with the TwentyTwelve theme, and when I click “report a bug”, the pop-up window with the bug-reporting form shows up, but it is empty. Firebug doesn’t show any error messages.
Has anyone else seen this happen? Any idea how to fix it?
