    This is something that the plugin doesn’t support. You could likely do some CSS changes with a text widget to get it to work according to how you want while respecting best practices. The forums at StudioPress.com would be a good place to start on getting some Streamline-specific suggestions on putting that together.

    What you’ve started to do does appear to work, though. The form action field would need to be replaced with wherever you’d want the link to go to.
