Remove Address Fields from Form

I would like to omit the Address Form fields from the map. By leaving this checkbox unchecked “Enable this option to add address fields to your maps” I believe the fields should not display. However, checked or unchecked, the fields display. Is there a short code needed to control the display of those fields?

Other than this, I really like the plugin. Will review.
