Registration page not working after initial set up

    I just configured Simple Membership Plugin as per documentation.
    But when I go to the Registration page via the Join Us link, there is no registration form to register.

    All I get is some text on the page below and a non-working button image.

    What do I need to do to make the registration form appear with basic registration inpu fields?
