Registration info for Quiz submission

I have installed the Word Press Quiz plugin WP-Pro-Quiz in my WordPress site.
Having tried a few other quiz plug ins, this is by far the best one for what we had in mind.
However, I cannot figure out how to ensure the visitors taking the quiz register in a form before/after submitting the completed quiz. Where are they to submit their name address/ email etc. The whole idea of the quiz is lead generation/information capture.
Am i missing something in the settings of the plugin?
Or do I have to add a contact form seperately
