Hello Jeff, thank you for the plugin!

I have WP 4.4 version, and premium Get Noticed Theme, and BuddyPress plugin. I don’t know where the issue is but the form does not appear in the Registration page:
    Register
and I have no Activate page. I can create one but don’t know what it should be on it as nothing appears.

I have to mention that in Dashboard/Settings/General the box is checked for “Anyone can register”.

I tried some things from other answers:
    – adding [raw][raw/]
    – adding other things inside the theme-my-login shortcode
    nothing worked…

The odd thing is that if my memory is not mistaking me, the first time I installed and activated TML the registration form was there, but was gone after a while.

Thank you for any tip or advice!

The pages are normal when BuddyPress is deactivated.
Once I activate it the Register page is again without the form. This means it’s a BuddyPress issue?

Not neccessarily a BuddyPress issue, but perhaps an incompatibility between the two.

It was the Get Noticed Theme. I installed the forum in a subdomain and problem was fixed with the same plugins installed on Twenty Sixteen theme (TML, BuddyPress and BBPress).
