Registration Form, Submission Form needed.

I’ve purchased the Contact Form Pro from BWS. Then I did ask them to make for me modified version to have video formats attached,just to add couple of more formats to the list of the media formats, then add one more field for Policy-Legal Agreement and the Radio button to check as agreed with the policy). They gave me the price $60, then in few days said $100. Then I start to negotiate, they added even more and made it $200 (as they said “especially for you”). They always try to humiliate, to put down, abuse in any way. So, I quit with such guys. Now I am looking for the new web host (since my BrainHost supports BWS, Best Worlds Software, then BrainHost having no any web support) but I still need this plugin (the Registration Form or Submission Form). If some one knows where can I get such form? Who is the vendor?
