Redirection Loops

Is anyone else ending up with redirection loops after renaming a page? I have had a few clients break their site since they changed the permalink for a page to something new and then tried to change it back, which ended up creating two entries in redirection, which ultimately caused a redirect loop since the old URL was redirecting to the new url, which was then trying to redirect back to the old URL. This seems like an easy fix if I knew where to look.
