    Could you tell me your WordPress, WooCommerce, PHP and plugin version you are use on your store?

    Please navigate to WooCommerce > Status > Logs and check for error messages related to the plugin.

    If there are any, I would very much appreciate it if you could send them in a feedback message.

    The issue seems to be with the fact that we are not using the default WooCommerce template for the My Account page. We are using the Impreza theme, however, it still outputs all of the other default WooCommerce account details so something in the way you are hooking into it is not translating to the theme.

    Is there a Shortcode that could be placed to output the Refund Request form? Otherwise I will have to share your plugin with the theme developers to see if something else could be done.

    Hi Jason

    There is no shortcode you could be able to display refund form on your site. Indeed, this could be a problem with the theme. Please check by switching to another theme, e.g. Store Front, before contacting your theme author.
