    In my questionnaire, radio buttons and check boxes don’t show up in Chrome. Any assistance is greatly appreciated.

    It looks like you may be running into a conflict. Do you have the same problem with all other plugins disabled and a default theme running? If you do, can you please open a ticket in our help desk so our team can best and most quickly assist you?
