radio and checkboxes don't show input (only in Safari)

I created a questionaire with Contact Form 7 plugin. see on
https://www.sanafit.ch/?page_id=52. Radiobuttons and checkboxes work without any problems in Firefox and IE but in Safari the radioboxes turn into checkboxes and the input is not visible even is is there and correct when I get the confirmation mail after submit. Any ideas what’s wrong and how to solve?
