In the Pro version, we have Leaderboard functionality.

can we add image upload field in form with name and email?

Nope, unfortunately there isn’t file upload field.
Here is the list of available custom field types:

• Text
• Textarea
• Email
• Number
• Telephone
• URL
• Select
• Checkbox

You can re-activate the plugins and change the theme back again after troubleshooting.
