Quiz, Survey or Form in Grid Format User Members List

Hi I am looking for a plugin that will allow me to create a grid/matrix style survey that uses members in my WordPress member database.

Basically I have a bunch of members and on a weekly basis I need to update certain statistics about these members. My grid would pull members from a certain member group and list them in the first column. The other columns would be for recording data – some may be radial buttons others need to be numeric fields and others columns will have a text answer.

The example below shows Bob and Sally who are members in my database. I need that information to be added to the survey based on a selection the user submitting the survey makes e.g. at the start of the survey they will choose a group which will determine which members are included in the survey.

The other columns will be the questions the users needs to answer. Like I said some columns will be text, some logic, some numeric.
