quiz plugin that allows results to be sent via email

Hi

i would like to know if there is any plugin that allows a user to complete a created quiz and submit the results to me via email. at the moment i have the quiz in page format and calculating the correct end result for the user/student can become very complicated and has to be done by a person on the receiving end. what i want at in the end is for the user to complete the quiz and once the user clicks in submit a email gets sent to me with all their answers to every question and a email address for me to sent the end result toooo.
