Quite good, a little bit restrictive in free version

  • Unfortunately there are no user defined fields which is what I need.
    EG a hostname field and url field.

    Adding a few variable fields would improve this addon immensely. The price is very fair.

    Yes exactly. An example of what I am doing (with Google Forms).
    If you added more optional fields, so that USERS could submit in Posts, it would make your addon very attractive as the price is right and its well designed.
