Questionnaire with yes / no checkboxes

  • Hey guys, do you know if it’s possible to create a questionnaire style form?

    eg. Do you like ice cream? YES | NO

    The user selects either yes or no for each question, and at the bottom of the form it totals the number of yes’s and no’s?
