Question pages keep reverting to 404

  • We have a server at Cloudways with the plugin installed and working well.

    Every few weeks, for no apparent reason we can tie it too, the question pages 404.
    This happens only on dwqa-question custom post type pages, and no other custom post type on the site nor on pages and posts.

    We are aware that visiting Permalinks in Settings recreates them and solves the problem.

    However, we would like to now have the problem happening in the first place!

    Do you have any ideas of what could be breaking the permalinks of questions in particular?

    Any insight would be much appreciated.
