Question before I download

Does this plugin include an email notification when a user uploads a file, and if so is any additional information that the user submitted along with the upload show up in the email? I currently have a file upload plugin, but the additional information that the user enters in doesn’t show up in the notification email, and that’s what my boss wants. For example, we operate a screen printing company. We need the ability for a potential customer to upload artwork, and enter in a description somewhere of what the artwork is for. We also need that description to show up in the notification email that we receive when the file is uploaded.
