Question about 'Add Contact Form'

If not, which plugin are you using?

No. I’m using the generic Add Contact Form that comes with WordPress. I’ve provided a dropbox link of a screen shot so you can see what I mean.

WordPress doesn’t come standard with Contact Forms.

That looks like Jetpack’s contact form module, so I recommend contacting them via https://jetpack.me/contact-support/
