Adding forms to our sliders is currently only possible by custom coding, what we don’t really suport. In a text layer you are able to instert your HTML code:

and the CSS and JS code could go for example into our Developer tab:
https://smartslider3.helpscoutdocs.com/article/1294-slider-settings-developer

So it can be done, but you probably would need to write customized codes for it.

actually i used shortcode and on text layer and it’s working without custom code , thank you
