Q & A Form

I am looking for a form that will allow me to post questions and after any text has been added to the form and submitted the answer is displayed. There will be multiple questions (up to 5) and the submit button could be after all questions are answered and all answers are displayed. We do not have to save the answers. We have not selected the theme or version we are going to use for this site.
