pull info from external database

I have a database that simply has codes in it for a promotion.
I want the user to enter an email address and then a code is displayed. Their email address is logged so it can’t be used twice and instead will just show the same code again.

The database part is easy, but how do I access it from a wordpress page form? Is there a plugin that can do this?
