Protocol relative urls are wrongly parsed

changin class-ss-util.php, line 197 to

$extracted_url = self::origin_host . ':' . $extracted_url;

solves the issue

Changing line 197 to:

$extracted_url = substr_replace( $extracted_url, self::origin_scheme() . '://', 0, 2 );

fixes the issue for me.

FYI: This is fixed in v2.0.4+ of Simply Static.
