Protect a PHP Form

If you are using it in the template assigned to the page then you can protect that page using Pages Access Level rule.

If you are not using page / post then you can use URL Groups Access Level rule to protect it.

Alternatively you can directly use membership plugin functions to protect it by writing code in the PHP form file.
