Proper Affiliate plugin

Hey guys

So I need the following functionality:

The website basically needs a form which allows to register some personal information about a visitor (this is required for its further manual registration procedure) – this is pretty straight-forward

So I need some module which allows to:
-register affiliates (every partner has to have his own account page)
-generate form with its personal embedded affiliate link

Extra info – we don’t need any super-automated payoffs with integrated paypal or any fancy wysiwyg widget editors !!!

Maybe someone could suggest some plugin or had any experience here and could share his/her thoughts ? Comments are welcome

Ive tried to look on the market – but there are tons of affiliate plugins !!!

So maybe someone could help me point at the right direction
