Profile Search Forms crashed

Hello,

Before upgrading to latest version of my theme Gwangi, Profil search and BP Maps for Members, we could search in the map the members choosing the date of birth and the location, and the profil male or female.

Now, we can not. I wonder if I can make Profil search and BP map works together in the same page like in the demo feature of the gwangi theme.
